29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. a. On or about November 22, 2019, the employer did not require the Leg Cut Out Baler Machine (Serial #977-06) to be deenergized and locked out when maintenance employees were clearing machine jams. Associated Hygiene Products, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(2) which was contained in OSHA inspection number 1137276, citation number 1, item number 3a and was affirmed as a final order on July 7, 2016, with respect to a workplace located at 301 Point Basse Avenue, Nekoosa, WI 54457.
