One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. On or about September 8, 2025, the employer did not adequately guard the rotating parts and nip points on the GDM Line 15 diaper machine (Serial # 0948M5).  An employee sustained fractures and lacerations to their hand as a result of contacting the in-going nip point created by the core forming drum.
