29 CFR 1910.178(l)(4)(ii)(B): Refresher training and evaluation was not provided to the operator when the operator was involved in an accident or near-miss incident. (a) Dock Door 1 - On or about December 9, 2019, a forklift operator was involved in an accident when he struck the top of a trailer, bending the frame. On December 10, 2019, the operator was observed using a forklift and had not received refresher forklift operator training.
