(a) On or about December 10, 2019, employees were working from a hydro mobile scaffold without adequate fall protection, thereby exposing employees to a fall hazard of approximately 12 feet.

(a) On or about December 10, 2019, an employee walked the concrete block wall between two hydro mobile scaffolds while enclosing a stairwell shaft. This exposed the employee to a fall hazard of approximately of approximately 10 8/10 feet on the inside and 8 6/10 feet on the outside of the poured concrete wall of the stairwell shaft.
