(weight of the worker, tools and equipment)  Casino Building, Facility Control Room: The A frame ladders observed during inspection did not have any manufacture data listed for load limits for employees to identify during inspection and use of ladder.  Employees are exposed to fall hazards.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was required to use the respirator in the workplace.  Casino interior building: An employee was required to use a 3M half face negative pressure respirator and a 3M N95 filtering face piece respirator while conducting spray painting and drywall sanding operations on the interior building walls and ceiling. The employee was not medically evaluated to determine if the respirator by itself could create a hazard for the employee.

29 CFR 1910.134(f)(1): The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT).  Casino, interior building: An employee was required to wear a  3M half mask negative pressure respirator and 3M N95 filtering face piece while spray painting walls and ceilings and during sanding of drywall.  The employer had not provided fit testing prior to using the respirators.  The employee was exposed to inhalation hazards.
