29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    On or about December 16, 2019, the employer failed to provide adequate guarding to protect employees from gaining access to the forming tools, forming slides and stamping press while operating the Finzer press machine. Employees were exposed to amputation and crushing hazards while standing in front of and operating the machine without the guards in place.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10.    On or about December 16, 2019, the employer failed to provide adequate guarding to protect employees from gaining access to the press frame while operating the Komatsu Servo Press. The openings in the guards were large enough to allow employees to access the point of operations, exposing them to amputation and crushing hazards.       Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.217(c)(2)(i)(d): It shall utilize fasteners not readily removable by operator, so as to minimize the possibility of misuse or removal of essential parts;    On or about December 16, 2019, the employer failed to ensure that appropriate fasteners were used to secure guards on the Komatsu Servo Press. Bungee cords and slide clips, which were easily removable, were used to secure the machine guarding in place, exposing employees to amputation and crushing hazards.      Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.217(d)(9)(iv): The employer shall provide and enforce the use of safety blocks for use whenever dies are being adjusted or repaired in the press.    On or about December 16, 2019, the employer failed to ensue that safety blocks were used when employees adjusted or repaired dies on the Komatsu Servo press machine. Employees were exposed to amputation and crushing hazards when performing adjustments or repairs without safety blocks being utilized.      Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
