29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    On or about February 19, 2020, the following employees were exposed to noise levels above 8-hour time-weighted average (TWA) of 85 dBA:     a.  A machinist was exposed to an 8 hour TWA of 87.1 dBA (Equivalent Dose of 66.8%).  Time sampled was 468 minutes.    b.  A machine operator was exposed to an 8 hour TWA of  88.7 dBA (equivalent dose of 83.9%).  Time sampled was 492 minutes.       c.  A machine operator was exposed to an 8 hour TWA of 91 dBA (Equivalent dose of 114.8 %).  Time sampled was 493 minutes.    Burnex Corporation did not establish and maintain an audiometric testing program to cover these employees.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.13(c).

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY)    On or about March 3, 2020, the building maintenance employee was not provided with the appropriate eye protection to use during the dilution of the cleaning agent Super Power.  The employee is provided with safety glasses that do not protect against splashes of chemicals.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19 (d).

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    On or about February 27, 2020, employees were in the process of transferring the corrosive material, Super Power, from the 50 gallon drum to a plastic container.  There was no eyewash station in the vicinity of the drums to quickly drench the eyes of the employees in the event of exposure to the corrosive substance.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
