29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  The employer does not ensure the protection equipment is used when employees are exposed to fall hazards while working above six (6') feet. the lack of fall protection was most recently observed on December 19, 2019, at the jobsite located at 19836 Silverside Dr. in Tinley Park, IL.  Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it taking to ensure compliance, including an explanation of how these steps protect its employees from fall hazards.  Domingo Garcia dba DNR Construction was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1, citation number 1a, item number 1242280 and was affirmed as a final order on 12/05/2017, with respect to a workplace located at 19524 Brookridge Drive in Tinley Park, IL.  Domingo Garcia dba Domingo Construction Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1318008, citation number 2, item number 1 and was affirmed as a final order on 08/01/2018, with respect to a workplace located at 11134 Third St. in Mokena, IL. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.  On or about December 19, 2019, at the above addressed jobsite, the portable ladder used to access the first level roof did not extend at least three (3) feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.  b.
