Case number P000006550, which occurred on or about October 30th, 2019, did not include a full description of the injuries, the body parts affected, and the object that directly caused injury to the employee on the OSHA Form 300. 2. Case number P000006570, which occurred on or about November 5th, 2019, did not include a full description of the object that directly caused injury to the employee on the OSHA Form 300. 3. Case number 0512266852, which occurred on or about November 7th, 2019, did not include a full description of the injuries, the body parts affected, and the object that directly caused injury to the employee on the OSHA Form 300. 4. Case number 0512266913, which occurred on or about November 14th, 2019, did not include a full description of the injuries, the body parts affected, and the object that directly caused injury to the employee on the OSHA Form 300. 5.
