29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):     On or about 8/10/2019 and at times prior, at 4723 Caspian Way, Davie, Florida, Building 22, the employer did not perform frequent and regular inspections to ensure that stairway openings on the 2nd floor were equipped with guardrails, and the subcontractor employees use fall protection when working from trusses, exposing employees to a fall hazard up to 19 feet.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):      On or about 8/10/2019 and at times prior, at 4723 Caspian Way, Davie, Florida, Building 22, employees and subcontractor employees who accessed the 2nd floor to clean the area after they finished working were exposed to a fall hazard of 9 feet to the ground floor due to the lack of guardrails around the stairway opening.     Prime Shell, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1146963, citation number 01, item number 001, issued on 7/29/2016, with respect to a workplace located at 11760 SW 248th Street, Miami, Florida 33032, which became a final order on 08/31/2016.
