29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or before January 7, 2020, where an employee was using a pneumatic nail gun to frame in the front porch of a new construction house, the employee's eyes were not protected, thereby being exposed to struck-by hazards.  b. On or before January 7, 2020, where an employee was using a pneumatic nail gun to secure sheathing to the roof trusses of a new construction house, the employee's eyes were not protected, thereby being exposed to struck-by hazards.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.  a. On or before January 7, 2020, where employees use an extension cord to provide electrical power, the outer jacket of the cord was damaged allowing the hot interior conductor to become exposed, thereby exposing employees to a 125-volt electrical contact hazard.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  a. Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a. On or before January 7, 2020, where employees were sheathing the roof of a new construction house, the employees were not protected from falls, thereby being exposed to a 20 foot fall hazard.  Samuel Moreno Construction, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1418882, citation number 1, item number 2 and was affirmed as a final order on November 5, 2019, with respect to a workplace located at 120 Ratcliff Lane, Plain City, OH 43064.
