29 CFR 1926.20(b)(2): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part; such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On January 10, 2020 at 1410 Mid Valley Drive in De Pere, the employer did not ensure that a competent person was performing frequent and regular inspections that could have recognized the fall from elevation hazard associated with employees accessing a roof edge and walking to a work area without guidelines installed to form a path between the open edge and work area warning lines.

29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines. On January 10, 2020 at 1410 Mid Valley Drive in De Pere, no guidelines installed between a roof work area warning line and the access/egress ladder; employees were walking between the warning line and unprotected roof edge sixteen feet above surface level without utilizing fall protection equipment.
