Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about November 10, 2021, at the worksite located at 830 Prosper Street in De Pere, Wisconsin; employees were exposed to fall hazards while removing an old HVAC unit without utilizing fall protection or a combination of fall protection exposing them to fall hazards greater than 10 feet.

29 CFR 1926.502(h)(1)(v): The safety monitor shall not have other responsibilities which could take the monitor's attention from the monitoring function. On or about November 10, 2021, at the worksite located at 830 Prosper Street in De Pere, Wisconsin; employees removing a HVAC unit was utilizing a safety monitor who had functions which took away from the monitoring in that he was communicating to employees by phone to assist in crane signals exposing employees to fall hazards greater than 10 feet.
