29 CFR 1910.28(b)(3)(ii): Protection from fall hazards - Holes. The employer must ensure: Each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems; a) On or about January 21, 2020 the employer did not ensure that employees performing rinse activities on the right side of the 110 plating line were protected from floor holes in that the raised work platform was placed 6" away from the rinse tank with a 6" depth to the ground.

29 CFR 1910.29(b)(1): Guardrail systems: The top edge height of top rails, or equivalent guardrail system members, are 42 inches (107 cm), plus or minus 3 inches (8 cm), above the walking-working surface. The top edge height may exceed 45 inches (114 cm), provided the guardrail system meets all other criteria of paragraph (b) of this section (see Figure D-11). a) On or about January 21, 2020 the top bar located on the rinse tank on the right side of the 110 plating line measured 34 inches from the raised work platform, exposing employees to a fall of approximately 4 feet 8 inches into the rinse tank. b) On or about January 20, 2020 employees were reaching over 34" high railing across the tank 3 feet to reach a water hose.
