29 CFR 1910.179(g)(1)(v): Pendant control box(es) were not clearly marked for identification of functions:  Fulton County Processing, LTD, Delta, OH: On or about January 22, 2020, the employer failed to ensure that the Shawbox overhead hoist, S/N  08F32740 pendant control boxes located at Pack line #2 were clearly marked for identification of function there by exposing employees to a struck by hazard.

29 CFR 1910.179(j)(2)(iii): A certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, was not maintained:  Fulton County Processing, LTD, Delta, OH, Slitter 2 Line: On or about January 22, 2020, the employer failed to ensure that a Shawbox overhead hoist, S/N 08F32740 hook and throat opening was inspected monthly.  Annual inspection revealed throat opening worn past 10 percent and should have replaced immediately there by exposing employees to a struck by hazard.

29 CFR 1910.179(j)(3)(iv): The employer did not ensure that worn, cracked or distorted parts such as pins, bearings, shafts, gears, rollers, locking and clamping devices were changed when needed.  Fulton County Processing, LTD, Delta, OH, Slitter 2 line: On or about January 22, 2020, the employer failed to replace a Shawbox overhead hoist, S/N  08F32740 hook that was found to be worn past 10 percent during an annual inspection conducted on June 10, 2019, there by exposing employees to a struck by hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Fulton County Processing, LTD, Delta, OH, Slitter 2 Line: On or about January 22, 2020, the employer failed to guard in going nip points on a powered conveyor. An employee measuring the width of a roll of steel suffered an amputation while the roll was moving down the conveyor exposing the employee to an amputation hazards.
