29 CFR  1910.147(c)(7)(i):The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  a) Fulton County Processing LTD, located at 7800 State Route 109 in Delta, Ohio:   On or about March 2, 2022, the employer did not ensure that employees who were tasked with changing out knives and tooling on the Paxson 24-inch slitter machine were trained in the skills required for the safe application, usage, and removal of energy controls. Employees were exposed to amputation and crushing hazards when removing and adding knives and tooling to the Paxson slitter machine, as they were not trained to isolate and/or render energy sources safe prior to performing servicing tasks.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Fulton County Processing LTD, located at 7800 State Route 109 in Delta, Ohio:  On or about March 2, 2022, employees were exposed to amputation injury in that the Paxson 24-inch slitter machine was not locked out when changing out knives and tooling. The employer did not ensure that energy control application steps to control the machine's electrical, pneumatic, and hydraulic energy sources were implemented prior to employees accessing the hazard area to dismantle and change out the cutting wheels. The employer did not:   -	Physically operate energy isolating devices to control energy sources and affix lockout device(s). -	Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. -	Verify that isolation and deenergization of the machine or equipment had been accomplished.

29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  Fulton County Processing LTD, located at 7800 State Route 109 in Delta, Ohio:   a) On or about March 2, 2022, the employer did not ensure the point of operation on the Paxson 24-inch Slitter Machine was guarded. Employees operating the machine were exposed to amputation hazards in that the in going nip point created between the cutting head and the in going steel was unguarded.   b) On or about March 8, 2022 the employer did not ensure the point of operation on the tension table of the Paxson 24-inch slitter machine was guarded. Employees were exposed to amputation and crushing hazards when engaging the tension table onto the steel during initial feeding.
