29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: On or about April 12, 2025, the employer did not ensure employees tasked with conducting repairs on slitter 1's scrap winders were provided with authorized employee LOTO training covering the hazardous energy sources, the type and magnitude of the energy, and the methods and means for energy isolation and control. Failure to provide training can lead to struck-by and crush-by hazards.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about April 12, 2025, the employer did not ensure that the company's energy control procedures were utilized to control hazardous mechanical movement of slitter 1's scrap winder door, when employees conduct repair tasks on the winders while working in the scrap area, exposing employees to struck by and crushed by hazards.
