29 CFR 1910.134(d)(3)(iii)(B)(2): The employer did not implement a change schedule for canisters and cartridges that was based on objective information or data that would ensure that canisters and cartridges were changed before the end of their service life:    M/V SAN PEDRO: Railway 4: A 3M 6300 elastomeric respirator with 3M P100 2097 cartridges used by an employee while welding and grinding for two weeks had not had the cartridges changed prior to the inspection. Employer had not implemented a change schedule for cartridges used based on objective information to ensure air contaminant exposure was not occurring. Employees were exposed to inhalation hazards.

29 CFR 1910.134(h)(3)(ii)(B): Respirator inspections did not include a check of elastomeric parts for pliability and signs of deterioration:    M/V SAN PEDRO: on Railway 4: A 3M 6300 elastomeric respirator with 3M P100 2097 cartridges used by employee conducting welding operations showed signs of deterioration.  The respirator no longer provided tight fitting face seal. Employees were exposed to inhalation hazards.

29 CFR 1910.134(g)(1)(ii): Corrective glasses, goggles or other personal protective equipment were worn in a manner that interfered with the seal of the respirator facepiece    M/V SAN PEDRO: on Railway 4: Employees performed MIG/TIG welding operations using a painter's sock underneath the full-face respirator which impeded a face-to-face seal. Employees were exposed to irritants and respiratory hazards such as filler material (i.e. paint coatings) vapors or fumes and metal fumes.

29 CFR 1910.134(h)(1)(i): Respirators issued for the exclusive use of an employee were not cleaned and disinfected as often as necessary to be maintained in a sanitary condition:    M/V SAN PEDRO: on Railway 4: A 3M 6300 elastomeric respirator with 3M P100 2097 cartridges used by employee during welding and grinding operations was not stored in sealable container or bag that ensured it was free from contaminants. In between moving down the scaffolding, moving to upper boat deck, while taking breaks and lunch, the employee had placed the respirator around his neck on top of contaminated clothing. Employee did not clean the respirator between uses or after completion of shift. Employees were exposed to ingestion and dermal hazards.
