29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  On or about February 4, 2020, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing the employees to the hazards associated with eye injuries.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a.  On or about February 4, 2020, at the above addressed jobsite, employees performing commercial roofing operations, thirty-two (32) feet above the ground, were not protected from falls, thereby exposing employees to fall hazards.  Dorados Exterior Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1355751, citation number 1, item number 2 and was affirmed as a final order on November 21, 2018, with respect to a workplace located at 326 Hemlock Lane, Oswego, IL.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a.  On or about February 4, 2020, at the above addressed jobsite, employees had been provided fall training, however, the employer failed to keep a written certification of record.
