29 CFR 1910.132(a): Protective equipment such as barriers were not used when necessary whenever hazards capable of causing injury and impairment were encountered. a. Concast Birmingham, Inc. dba Concast Metal Products located in Wakeman, Ohio: On or about January 31, 2020 the employer did not ensure an employee who was attempting to clear bronze from the dies on the #6 casting furnace located in the foundry, was protected from molten metal contact through the use of barriers prior to removing the casting die assembly. As a result, molten metal exited the furnace and contacted the employee's right leg and ankle causing a serious burn injury.

The employer did not ensure that respirators are cleaned and disinfected using the procedures in Appendix B-2 of this section, or procedures recommended by the respirator manufacturer, provided that such procedures are of equivalent effectiveness. a. Concast Birmingham, Inc. dba Concast Metal Products located in Wakeman, Ohio: On or about March 19, 2020 the employer did not ensure powered air purifying respirators worn by employees working on or near the melting furnaces were clean and sanitary. Wipe samples obtained from an employee's respirator, located in the employee's locker, showed that it was contaminated with cadmium, arsenic, and lead.

The engineering controls and work practices associated with the employee's job assignment; 4. The purpose and a description of the medical surveillance program required by paragraph (l) of this section; 5. The contents of this section and its appendices; and 6. The employee's rights of access to records under �1910.1020(e) and (g).
