29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: Worksite located at 760 W. 46th St., Kansas City, Missouri- Employees exposed to fall hazards were using harnesses and lanyards that had not been inspected for wear and damage prior to usage by the employees. The lanyards were in pour condition and covered in plastering materials. Lanyards make, model, or year it was manufactured was not able to be identified due to wear.
