29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:  a) Warehouse Floor: On or about 03/06/2020, employees working in the packing and storage areas of the warehouse were not protected from potential slip hazards in that, rainwater accumulated on the flooring.  b) Warehouse Area, Chemical Mix Area: On or about 03/06/2020, employees were exposed to potential slip hazards in that, a mixture of water and chemicals created slick patches on the flooring throughout the chemical mixing area.  c) Warehouse Area, Outside Women's Restroom: On or about 03/06/2020, the employer exposed employees to a trip hazard in that an extension cord, used as a power source for a water cooler was plugged into a wall outlet. The extensions cord laid outside of the women's restroom door on the floor, creating a trip hazard.  d) Warehouse Area - Near Sprinkler System: On or about 03/06/2020, the employer exposed employees to potential trip and /or slip hazard because large amounts of trash and boxes were scattered about the flooring in the work area.  e) Warehouse Area, Men and Women Restrooms - On or about 03/18/2020, the employer exposed employees to the slip and sanitary health hazards in that, deficient plumbing conditions caused leaks that spilled onto the flooring; in both the male and female restrooms.  f) Warehouse - Side Dock Area Rollup Doors - On or about 03/06/2020, employees working in the packing area of the facility were potentially exposed to a fall hazard of approximately four (+) ft. Although employees were allowed to use the dock/bay door opening to get fresh air, railing to prevent falls from dock/bay; was not installed.  g) Warehouse - Roof Area - On or about 03/06/2020, employees were not protected from fall hazards in that, the use of conventional fall protection devices were not enforce by the employer. Employees who accessed the roof area when performing servicing and/or maintenance on the roof were exposed to a fall of approximately 33 feet high from the roof to ground level.

The doorway which can be mistakenly used as an exit, leads to an enclosed courtyard.     b) Warehouse - Near Maintenance Shop Area - On or about 03/06/2020, employees were exposed to fire hazards in that a door located near the maintenance shop was not marked "Not an Exit". 29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:   a) Warehouse Area, Chemical Mixing Area Exit Door and in proximity to the Back Roll-up Door/ Ramp - On or about 03/06/2020, the employer exposed employees to struck-by and/or fire hazards in that the employer failed to ensure that compressed gases, such as but not limited to, propane, were properly stored.  Employees who worked in the proximity to unchained propane cylinders were not protected should the cylinders rupture, be hit by a powered industrial vehicle, or fall and become a projectile.
