29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  Worksite located at 10416 N Herlena Ave, KC MO - Employees were exposed to an eye impact hazard in that the employees were using pneumatic nail guns and other tools that could cause employees to be injured due to eye impact and were not wearing eye protection. This was documented on or about 03/11/2020.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  At the residential worksite located an Willow Way Ct in St Joseph, MO -- Employees were using an orange extension cord to power an electric portable skill saw. This extension cord did not have a ground pin. This exposed employees to electrical shock and/or burns. This was documented on or about 03/11/2020.

At residential worksite located at 10416 N Herlena Ave, KC MO -- Employees were exposed to fall hazards in that the ladder did not extend at least 3 feet above the landing surface.
