29 CFR  1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of this section and did not institute a training program and ensure employee participation in the program:   At a jobsite located at 2220 Lakewood Boulevard, Topeka, Kansas, on or about September 27, 2025, employees  installing sheet metal roof were exposed to fall hazards of approximately 25 feet from the roof to the concrete flooring below.  The employer failed to establish a training program on the use of fall protection equipment, the recognition of potential fall hazards, and requirement for fall protection use as it relates to installing sheet metal roof.  The result of this failure was evident on this day when an employee who did not have fall protection fell approximately 25 feet suffering fatal injuries.

Sanchez Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1761532, citation number 1, item number 1, and was affirmed as a final order on September 9, 2024, with respect to a workplace located at 22009 W 57th Terr, Shawnee, KS 662266.
