29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   The employer did not provide the Occupational Safety and Health Administration certification and documentation of abatement as required for Citation #1, Items #1 & #2 issued April 2, 2020.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Employees were exposed to fall hazards.  Employees engaged in residential roofing activities, walking and working on a 6:12 pitch roof, were exposed to fall hazards of approximately 11 feet from the eave of the residence to the ground below in that fall protection was not utilized

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Employees engaged in residential roofing activities were exposed to falls from approximately 11 feet, in that the aluminum ladder used to access the roof did not extend 3 feet from the eave of the roof and was not secured to the building
