29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At the jobsite located at 2218 Watercress, Andover, KS, employees engaged in residential roofing activities were exposed to struck-by hazards, employees were not wearing any eye protection while using pneumatic nail guns.  Jesus Quevedo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number1499350, citation number 1, item number 1 and was affirmed as a final order on 02/09/21 with respect to a workplace located at  3503 S Lori, Wichita KS 67213.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Employees engaged in residential roofing activities at the jobsites located at 2218 Watercress, Andover, KS were exposed to fall hazards of approximately 17 feet from the roof to the ground below in that fall protection was not provided.  Jesus Quevedo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 (b)(13), which was contained in OSHA inspection number 1500409, citation number 1, item number 1, and was affirmed as a final order on 02/09/21, with respect to a workplace located at 4932 Peregrine St, Wichita, KS .  Jesus Quevedo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 (b)(13), which was contained in OSHA inspection number 1499350, citation number 2, item number 1, and was affirmed as a final order on 02/09/21, with respect to a workplace located at 3503 S Lori, Wichita, KS. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Employees engaged in residential roofing activities at the jobsites located at 4932 Peregrine St, Wichita, KS, were exposed to falls from approximately 10 feet from the roof to the ground below, in that the aluminum ladder used to access the roof did not extend 3 feet from the eave of the roof.
