29 CFR 1904.4(a): Each employer required by this Part to keep records of fatalities, injuries, and illnesses did not record each fatality, injury, and illness (as required under 29 CFR 1960.66(b)):  Location: Rocky Ridge Boarding School, Kykotsmovi, AZ.   (a)   On March 30, 2020 the employer/agency became aware that an employee died on March 27, 2020 from a presumed occupational illness.  By May 22, 2020 the employer had not recorded the fatality on the OSHA 300 Log. The employee had been in direct contact with a coworker who was positive for CoVID-19 on multiple occasions for at least 6 days, including a budget meeting on March 13, 2020 in a small office space.    (b)     On or about April 3, 2020 the employer/agency became aware that an employee had tested positive for a presumed occupational illness.  By May 22, 2020 the employer had not recorded the illness on the OSHA 300 Log. The employee had been in direct contact with a coworker who was positive for CoVID-19 for several hours a day for at least 7 days, as a routine part of his work. Both workers open the school together each morning before other staff and students arrive.  (c)    On or about April 7, 2020 the employer/agency became aware that an employee had tested positive for a presumed occupational illness.  By May 22, 2020 the employer had not recorded the illness on the OSHA 300 Log. The employee had been in direct contact with a coworker who was positive for CoVID-19 for at least 15 minutes a day for at least 7 days, as a routine part of his work.  (d)   On or about April 7, 2020 the employer/agency became aware that an employee had tested positive for a presumed occupational illness.  The employee had been in direct contact with a coworker who was positive for CoVID-19 for at least 15 minutes a day for at least 7 days, as a routine part of his work.  (e)     On or about April 7, 2020 the employer/agency became aware that an employee had tested positive for a presumed occupational illness.  By May 22, 2020 the employer had not recorded the illness on the OSHA 300 Log. The employee had been in direct contact with a coworker who was positive for CoVID-19 on multiple occasions for at least 6 days, in addition to frequently touching door knobs, handles, and surfaces contacted by both workers.
