29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:   A)     Morral Companies, LLC, located at 132 Postle Avenue, Morral, OH 43337: On or about April 16 and April 17, 2020, the employer did not ensure that employees were protected from fall hazards while working on top of the Lawn Mixer located in the Liquid Department. Employees conducted activities such as, but not limited to, removing the manway/latch from the top of the mixer and working on the steam valve on top of the mixer. Employees were not protected by guardrails, safety net systems, or by personal fall arrest systems. Employees were exposed to fall hazards of approximately 14 feet from the mixer to the floor below and to fall hazards of approximately 7 feet from the top of the mixer to the mixer bottom inside the mixer.
