Employees conducted activities such as, but not limited to, removing the manway/latch from the top of the mixer and working on the steam valve on top of the mixer. Employees were not protected by guardrails, safety net systems, or by personal fall arrest systems. Employees were exposed to fall hazards of approximately 14 feet from the mixer to the floor below and to fall hazards of approximately 7 feet from the top of the mixer to the mixer bottom inside the mixer.
