On or about May 15, 2020 at 470 N Market St, in Logan, Ohio employees were not trained to recognize hazards while working on the 6/12 pitch roof of the pool house exposing employees to a 10 foot fall hazard.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a. On or about May 15, 2020 at 470 N Market St, in Logan, Ohio employees were installing shingles on a 6/12 pitch roof on the pool house with no fall protection exposing employees to a ten foot fall hazard.
