29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  a) Sanchez Construction LLC, at a jobsite located at 6823 Streamside Drive, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer did not ensure employees were protected from electrical contact while engaged in residential construction activities. Employees were using an extension cord with the outer insulation jacket pulled away from the strain relief of the male end of the cord, thereby exposing employees to a 115 volt electrical contact hazard.

29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.   a) Sanchez Construction LLC, at a jobsite located at 6823 Streamside Drive, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer failed to initiate and maintain a safety and health program such as but not limited to an electrical safety program and a fall protection program for employees conducting framing work.  Sanchez Construction LLC (Jorge Gomez) was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1423718, citation number 1, item number 1 and was affirmed as a final order on December 4, 2019 with respect to a workplace located at Lot 40, 96950 Scarlet Oak Drive, Hilliard, OH 43026.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  a) Sanchez Construction LLC, at a jobsite located at 6823 Streamside Drive, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer failed to train employees on the common hazards found in residential construction or the regulations required to control or eliminate the hazards. Employees conducting framing work were exposed to electrical contact hazards and fall hazards.   Sanchez Construction LLC (Jorge Gomez) was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1423718, citation number 1, item number 2a and was affirmed as a final order on December 4, 2019 with respect to a workplace located at Lot 40, 96950 Scarlet Oak Drive, Hilliard, OH 43026.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a) Sanchez Construction LLC, at a jobsite located at 6823 Streamside Drive, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer did not ensure employees were protected from falls while engaged in residential construction activities. Employees were sheathing and framing the second story without any means of fall protection, thereby exposed to fall hazards of up to 14 feet 4 inches.  b) Sanchez Construction LLC, at a jobsite located at 6823 Streamside Drive, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer did not ensure employees were protected from falls while engaged in residential construction activities. Employees were framing where the basement opening was unguarded, thereby exposed to fall hazards of up to 10 feet.  c) Sanchez Construction LLC, at a jobsite located at 6823 Streamside Drive, Galena, Ohio 43021: On and/or preceding May 26, 2020, the employer did not ensure employees were protected from falls while engaged in residential construction activities. Employees were framing the second story where the stairway was unguarded, thereby exposed to fall hazards of up to 10 feet.  Sanchez Construction LLC (Jorge Gomez) was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1423718, citation number 1, item number 3 and was affirmed as a final order on December 4, 2019 with respect to a workplace located at Lot 40, 96950 Scarlet Oak Drive, Hilliard, OH 43026.  Sanchez Construction LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1258088, citation number 1, item number 1 and was affirmed as a final order on October 23, 2017, with respect to a workplace located at 4 Burroughs Drive Ashville, OH 43103.
