29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  a- Mobile Scaffold:  An employee beneath a worker on the 3rd scaffold section was exposed to an over head impact hazards as head protection had not been utilized.  Condition noted or about 06/17/2020.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  a- Mobile Scaffold:  An employee climbing the 2 and 3rd sections was exposed to fall hazards of approximately 12 & 18ft as a means of scaffold access and egress had not been utilized. Condition noted or about 06/17/2020.

29 CFR 1926.452(w)(6):  Employees shall not be allowed to ride on scaffolds unless the following conditions exist:  a- Mobile Scaffold: An employee engaged in painting activities from the third scaffold section of tubular welded frame mobile with a height ratio of 1 to 3 was exposed to fall hazards of approximately 18ft as the scaffold was moved while conveying an employee. Condition noted or about 06/17/2020.
