29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition: A.) Glen Gery Corporation, grinding room - On or about 6/17/20, and at times prior thereto, the floor was not cleaned frequently enough to ensure accumulations of shale were not on the floor areas where employees walked. The condition created a slip and fall hazard. These conditions existed on the floor in areas that included but were not limited to the floor area by the bucket elevator. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts: A.) Glen Gery Corporation, grinding room/Bucket Elevator Inspection Port Area - On or about 6/17/20, and at times prior thereto, the inspection port for the bucket elevator was not guarded to prevent employees from being able to place hand/arms into the moving parts of the machinery. The rubber cover was not secured to prevent it from being opened without any special tools or knowledge. In addition, the rubber cover was not so designed and constructed as to prevent the employees from having any part of their body in the danger zone during movement of the machine. The moving parts included but were not limited to the moving bucket. NO ABATEMENT CERTIIFCATION REQUIRED:
