29 CFR  1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:   Phoenix Indian Medical Center located at 4212 N 16th St, Phoenix, AZ 85016:  On or about September 24, 2020, employees wore cloth face masks when surgical masks or N95s were required to protect against the SARS-CoV-2 virus while providing care to COVID-19 patients.

29 CFR  1910.132(f)(1)(v): Employee(s) required to use PPE by this section were not trained to know the proper care, maintenance, useful life, and disposal of the PPE:   Phoenix Indian Medical Center located at 4212 N 16th St, Phoenix, AZ 85016:  On or about July 10, 2020 through August 10, 2020, employees were not trained on the reuse of isolation gowns, exposing employees to the SARS-CoV-2 virus from cross contamination during storage between uses.

29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not implemented or updated as necessary to reflect those changes in workplace conditions that affect respirator use.  Phoenix Indian Medical Center located at 4212 N 16th St, Phoenix, AZ 85016:  On or about August 26, 2020, the employer did not develop and implement a written respiratory protection program with worksite-specific procedures for respirator use that included all provisions in 29 CFR 1910.134(c)(1)(i) - (ix). The employer required employees to wear respirators throughout their shifts while exposed to suspected or confirmed positive COVID-19 patients, however procedures for the use of respirators protection during pandemic and surge conditions were not included.
