29 CFR 1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) MAINTENANCE SHOP: Three relocatable power taps were "daisy chained" to a fourth power tap, exposing employees to electrical shock and fire hazards. b) RADIOLOGY BREAKROOM: A 15A Dayton relocatable power tap and a 15 foot extension cord were "daisy chained", exposing employees to electrical shock and fire hazards. c) POWER PLANT: A receptacle box with knockouts, designed for permanent mounting, was provided with a flexible cord. The receptacle box and flexible cord were used as a power tap to provide electrical power to two automatic blowdown relays of pneumatic thermostat systems.
