29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter:     Machine shop welding table:  An annual fit test had not been provided to an employee wearing a tight fitting respirator while welding.  Employee was exposed to respiratory hazards.

29 CFR 1910.134(g)(1)(i)(A): The employer did not prohibit the growth of facial hair of employees using tight fitting face piece respirators which would prevent a seal between the face piece and the face:  Machine shop welding table:  An employee with facial hair was wearing a dual cartridge tight fitting respirator while tig welding stainless steel.  The employee was exposed to respiratory hazards.

29 CFR 1910.252(b)(2)(iii): Workers or other persons adjacent to the welding areas were not protected from the rays by noncombustible or flameproof screens or shields or required to wear appropriate goggles.    Machine shop welding table:  Employees were working near and around welding operations without eye protection from arc welding rays.
