On February 20, 2024, main production building: The Southwestern Industries, Inc. Trak DPMRX7 Milling machine, was operated without a chuck guard.  Employees were exposed to rotating part hazards.  C. On February 20, 2024, secondary production tent: The 16" Lodge & Shipley Model "A" lathe was operated without a chuck guard.  Employees were exposed to rotating part hazards.

Employee was exposed to embolism and struck by hazards.

On February 20, 2024, main production building at Compressed Air Station #1: The Hydmech S-23P band saw blade guard was not adjusted to the size of the material being cut. Employees were exposed to cut and laceration hazards.  Pacific Shipyards International was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1487208, citation number 1, item number 1 and was affirmed as a final order on October 27, 2020, with respect to a workplace located at Pier 24.
