29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  Location: West side of building @ 210 Harrison Ave. Boston, MA 02111  On or about 07/09/20 employees were working from a 2 point suspended Spider scaffold, when one end of the scaffold would not stop lowering.  The employee could not stop the scaffold from the right hand side of the scaffold lowering. Employees were trapped and had to be rescued by the Police & Fire Department breaking an apartment window.
