29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    a) On or about July 21, 2020 employee were accessing the tops of Presses 20-25 to rig molds and/or perform maintenance activities without the use of fall protection that exposed them to fall hazards up to 15 ft. from the floor level.     b) On or about July 21, 2020 the elevated platform on the injection side of Press 20 was not guarded to prevent employee exposure to a fall hazard.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    a) On or about July 21, 2020 the energy control procedures for injection molding presses 20-25 did not specifically outline the techniques to release and/or relieve stored energy nor how to verify energy was effectively dissipated prior to the start of servicing and/or maintenance activities.     b) On or about July 21, 2020 the energy control procedures for the injection molding presses 20-25 did not specifically outline the magnitudes of the energy sources powering the equipment.     c) On or about July 21, 2020 the energy control procedures for the injection molding presses 20-25 did not specifically outline the procedural steps for the transfer of lockout devices.
