Employees entered the OCCScreen machine through a small access hatch to dislodge debris collected within the spindles or discs. The screening machine utilized electricity to rotate the spindles. The employees moved within the screening machine upon the unstable surface of the spindles and discs. Gaps existed between spindles and discs that were large enough to entrap employee's feet or legs. This condition exposed employees to an entrapment hazard.
