29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Rebar Building: On or about July 23, 2020, employees were exposed to amputation and crushing hazards by an unguarded point of operation on the Ironmaster rebar bending machine. b) Rebar Building: On or about July 23, 2020, employees were exposed to amputation and crushing hazards by an unguarded point of operation on the KRB #8 rebar bending machine, stock number ST97-B0328. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
