29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) On July 29, 2020 at the worksite located at 430 Main Street in Huron, OH: The employer did not have a plan or program to identify job site hazards which assist employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) On July 29, 2020 at the worksite located at 430 Main Street in Huron, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing roofing installation from elevated surfaces.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system:  a)  On July 29, 2020 at the worksite located at 430 Main Street in Huron, OH the employer did not assure employees were protected from fall hazards while performing roofing activities on a church.  The employees were exposed to approximately a 10 foot fall to the ground below.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a)  On July 29, 2020 at the worksite located at 430 Main Street in Huron, OH: the employer did not ensure employees were protected from fall hazards while working from a ladder jack scaffold.  The employees were exposed up to a 20 fall to the ground below during roofing operations of a church. No form of fall protection was in place.     On Top Roofing & Renovations, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.451(g)(1)(i) which was contained in OSHA inspection number 1236018, citation number 1, item number 3 and was affirmed as a final order on September 5, 2017 with respect to a workplace located at 35290  Detroit Road in Avon, Ohio.
