Located at 7070 Ringbill Loop in Sunbury, Ohio: On or about August 11, 2020, an employee was in an approximately six foot deep trench installing sanitation pipe. The benching of the trench walls did not provide adequate cave in protection, thereby exposing the employee to crushed by hazard.    Tysinger Construction LLC was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 1330331, citation number 1, item number 3, and was affirmed as a final order on 08/23/2018, with respect to a workplace located at 79 Cloverhill Drive in Galloway, Ohio.
