29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 19, 2020, during the removal and installation of roofing material on a two story residential home located at 12541 Lake Avenue, Lakewood, Ohio, the employer did not ensure employees working from the roof were protected from a fall of approximately 20 feet, by utilizing a guardrail system, safety net system and/or personal fall arrest system. ALL SIDES CONSTRUCTION, LLC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1268494, CITATION NUMBER 01, ITEM NUMBER 01, AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 18, 2017, WITH RESPECT TO A WORK PLACE LOCATED AT 17112 RIVERSIDE DRIVE, LAKEWOOD, OHIO.
