29 CFR  1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :  At a worksite located at 1301 Stahl Drive, Warrensburg, MO: Employees were exposed to occupational noise hazards. This was demonstrated on August 19, 2020 when employees at the facility were included in the employers hearing conservation program. As part of the program the employer failed to ensure that all employees were being provided new and audiogram annually.

29 CFR  1910.132(d)(1):The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  At a worksite located at 1301 Stahl Drive, Warrensburg, MO: Employees are exposed to physical safety and health hazards, including but not limited to serious irritation to the eyes, mucus membranes, respiratory system, and skin. This was most recently demonstrated on August 19, 2020, where workers were permitted to perform activities in the workplace that had not been assessed for any potential hazards. The employer failed to ensure that a PPE assessment had been performed at the facility that addressed all present hazards, or those likely to be present.

29 CFR  1910.178(l)(4)(iii):An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:  At a worksite located at 1301 Stahl Drive, Warrensburg, MO: Employees were exposed to struck by hazards. This was demonstrated on August 19, 2020 when employees were operating forklifts and had not been provided refresher training at least once every three years.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage to cord and plug connected equipment that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:  At a worksite located 1301 Stahl Drive, Warrensburg, MO: Employees were exposed to electrical shock hazards. This was demonstrated on August 19,2020 when a pedestal fan (TPI Industrial) was being used to provide cooling for employees in the work area. The pedestal fan was plugged into a receptacle directly and in use, but was was missing its ground pin.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  At a worksite located at 1301 Stahl Drive, Warrensburg, MO: Employees were exposed to toxic and/or hazardous substances. This was demonstrated on August 19, 2020 when employees were exposed to chemical substances including, but not limited  to Cobra 15N, Isoset I and II, CRC Knock'er loose, Sprayon LU711, Sulfer Dioxide (SO2) Sodium Hydoxide 50% without having effective training on the hazards of the chemicals and substances they are exposed to during the production process. The employer failed to train these employees on the specific hazards associated with these substances prior to their use in the facility.
