(A)  On or about August 27, 2020, at 16434 Heather Lane, Middleburg Heights, Ohio 44130, the employer did not ensure that employees operating nail guns used appropriate eye protection to protect them from flying particles.

29 CFR 1926.501(c): When employees were exposed to falling objects the employer did not require employees to wear hard hats and did not implement measures to prevent objects from falling from higher levels as set forth in (c)(1)-(3) of this section:    (a) On or around August 27, 2020 at 16434 Heather Lane, Middleburg Heights, OH 44130, employees on the ground level completing clean-up for roofing operations were not wearing hardhats, nor were any protective measure implemented to protect employees from falling objects.
