29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  Employees conducting battery charging operations in the South shipping battery charging room were exposed to acids in that the employer had not provided eye or face protection.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.  Employees in the south shipping battery charging room were exposed to chemical burns from acid, in that the employer did not have hand protection available for use during battery charging operations.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  Employees conducting battery charging activities, in the south shipping battery charging area, were exposed to acids in that the eyewash and shower station were unserviceable because it had no water pressure.

29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control  In the Distribution Center (DC), the employer failed to develop adequate procedures for locking and tagging out Laced Conveyor M-108-A in that the de-energization step did list specific requirements for testing the machine to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures, exposing employees to caught in hazards.

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished.  In the distribution center (DC), the laced conveyor M-108-A was incorrectly locked and tagged out in that no verification step was taken to ensure that the equipment was de-energized, exposing employee to a caught in and amputation hazard.
