29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: A.) Ideal Systems USA, Inc., 18 West 18th Street, Erie, PA 16501, roof, north side of the structure, on or about September 24, 2020 and times prior thereto: Employees were performing roofing activities that included, but was not limited to, removing debris. The top rail of the guardrail system was not installed. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge: A.) Ideal Systems USA, Inc., 18 West 18th Street, Erie, PA 16501, roof, west side of the structure, on or about September 24, 2020 and times prior thereto: Employees were performing roofing activities that included, but was not limited to, removing debris. The top rail of the guardrail system was deficient in that it was equipped with caution tape and would not meet strength requirements. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: A.) Ideal Systems USA, Inc., 18 West 18th Street, Erie, PA 16501, roof of the structure, on or about September 9, 2020 and times prior thereto: Employees were performing roofing activities that included, but were not limited to, cleaning and removal of debris during roofing activities. Employees were exposed to a fall hazard of over 25-feet. Employees were not utilizing a means or method of fall protection. Ideal Systems USA, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1355738, citation number 1, item number 2 and was affirmed as a final order on December 14, 2018, with respect to a workplace located at 2711 Columbia Blvd., Bloomsburg, PA 17815. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
