29 CFR 1910.67(c)(3): "Electrical tests." Electrical tests shall be made in conformance with the requirements of ANSI A92.2 - 1969, Section 5. However, equivalent DC voltage tests may be used in lieu of the AC voltage test specified in A92.2 - 1969. DC voltage tests which are approved by the equipment manufacturer or equivalent entity shall be considered an equivalent test for the purpose of this paragraph (c)(3). a) Elect General Contractors, Inc., at a workplace located at 1619 Pershing Road, Zanesville, Ohio 43071: On and/or preceding September 17, 2020, the employer did not ensure the electrical tests were conducted on vehicle-mounted elevating and rotating work platforms. Employees worked from elevated bucket truck number 84 near overhead electrical lines without dielectric tests being conducted within the previous 12 months. b) Elect General Contractors, Inc., at a workplace located at 1619 Pershing Road, Zanesville, Ohio 43071: On and/or preceding September 17, 2020, the employer did not ensure the electrical tests were conducted on vehicle-mounted elevating and rotating work platforms. Employees worked from the Digger Derrick truck number 92 near overhead electrical lines without dielectric tests being conducted within the previous 12 months. c) Elect General Contractors, Inc., at a workplace located at 1619 Pershing Road, Zanesville, Ohio 43071: On and/or preceding September 17, 2020, the employer did not ensure the electrical tests were conducted on vehicle-mounted elevating and rotating work platforms. Employees did not inspect fall protection equipment and used a lanyard that was damaged while working from an elevated bucket.

Bucket truck number 24 was not inspected each day and removed from service when unsafe conditions existed. b) Elect General Contractors, Inc., at a workplace located at 1619 Pershing Road, Zanesville, Ohio 43071: On and/or preceding September 17, 2020, the employer did not ensure the vehicle-mounted material handling devices and other mechanical equipment were inspected each day.
