29 CFR  1926.502(b)(3):Guardrail systems shall be capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge.  a. Located at or near 6050 East Broad Street, Columbus Ohio, 43213:  On or about April 30, 2025, the employer failed to provide employees with a safe means of accessing and exiting the ladder for the trench shielding box, as the makeshift platforms guardrails could not withstanding at least 200 pounds of force in any direction along the top edge.  This exposed the employees to fall and caught-in-between hazards as a result.

29 CFR  1926.652(g)(1)(ii):Shields shall be installed in a manner to restrict lateral or other hazardous movement of the shield in the event of the application of sudden lateral loads.  a. Located at or near 6050 East Broad Street, Columbus Ohio, 43213:  On or about April 30, 2025 and times prior,  employees were exposed to cave-in and crushing hazards while working inside a trench where the employer did not ensure that the trench box was installed to restrict lateral movement. A 3-4 foot gap was between the trench shield walls and the face of the trench on both sides which exposed employees to a struck-by hazards in the event of lateral movement coming from a sudden shift in lateral loads.

Located at or near 6050 East Broad Street, Columbus Ohio, 43213:  On or about April 30, 2025, the employer did not ensure that the ladder on the jobsite was inspected by a competent person for visible defects prior to being used. The employees were using a ladder to enter/exit a trench that had bent and missing rungs. These conditions exposed the employees to falls of up to 12 feet.

29 CFR  1926.651(k)(1):Daily inspections of excavations, the adjacent areas, and protective systems shall be made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. An inspection shall be conducted by the competent person prior to the start of work and as needed throughout the shift. Inspections shall also be made after every rainstorm or other hazard increasing occurrence. These inspections are only required when employee exposure can be reasonably anticipated.  a. Located at or near 6050 East Broad Street, Columbus Ohio, 43213:  On or about April 30, 2025 and times prior, the employer failed to ensure that the designated competent person conducted daily inspections of the jobsite. Employees were working inside trench shielding boxes within an excavation approximately 12 feet deep, without a competent person inspecting the excavation, surrounding areas, or protective systems. As a result, workers were exposed to struck-by and caught-between hazards.   Elect General Contractors Inc,  was previously cited for a violation of this Occupational Safety and Health standard 29 CFR  1926.651(k)(1), which was contained in OSHA inspection number 1791633, citation number 1, item number 1 and was affirmed as a final order on April 1, 2025, with respect to a workplace located at 2595 Brice Rd, Reynoldsburg, OH 43068.

When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours.  a. On or about June 3, 2025, the employer failed to provide the OSHA 300A's for past three years, the OSHA 301's or equivalent for last 6 months, and the OSHA 300 log for 2025 for recordable injuries and illnesses when requested by an OSHA representative.
