On or about 9/15/2020, Universal Packaging Corporation did not examine the electric equipment and ensure it was free from recognized hazards that are likely to cause death or serious physical harm to employees. Employees were operating a Paco labeling machine, serial number 911-1. The machine had a positive and neutral wire running out of metal conduit. The wires were spliced outside of the conduit and hanging below the machine; they were not enclosed in conduit or a junction box. The wires carried approximately 115 VAC, exposing the employees to electrical hazards.

On or about 9/15/2020, Universal Packaging did not ensure all electrical equipment was installed in a neat and workmanlike manner. 29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains were not enclosed that were less than seven (7) feet above the floor or platform. On or about 9/15/2020, employees from Universal Packaging Corporation were operating two Paco labeling machines, serial #s 911-1 and 964-4, on the manufacturing line. The machines had partially unguarded chains and sprockets, which were less than seven feet above the floor. Universal Packaging Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.219(f)(3), which was contained in OSHA inspection number 1485323, citation number 1, item number 1 and was affirmed as a final order on 9/08/2020, with respect to a workplace located at 3645 Tree Court Industrial Boulevard, Kirkwood, MO.
